Clinical Risk Assessment and Management

This policy outlines the Trust’s requirements for the assessment and management of clinical risk; that it must be adequately considered, recorded and communicated, and outlines the principles involved and the competencies required. Adequate consideration of the assessment and management of risk necessitates an approach which is structured, evidence based and consistent. A standardised risk assessment is given on Trust Digital Care Record systems which will be used by all those working within an area which has not identified the need for, or had approved an alternative tool or structured prompt. The process by which an alternative risk tool or structured prompt is identified and approved for use is outlined.
