• The specifics of your enquiry
• The team to which your enquiry relates
• Your details so we can respond to you

Please be aware that if your enquiry relates to the care of someone else, we may require the person’s consent. We will not require consent if, you have their written permission, you are the parent or guardian of a child under 16, you are next of kin and the person does not have capacity to enquire themselves, or you are next of kin and the person has sadly passed away. On receiving the enquiry the Patient Experience Team will contact you with a consent form if this is required.
