We support people who have a learning disability and/or autism who are struggling with their mental health, and who are at risk of an inpatient admission.

How we can help
We offer support for up to six weeks if you have a learning disability and/or autism, are struggling with your mental health and are at risk of an inpatient admission.
We will work closely with you, your family, your care team and other services to understand your needs, review your care plan and provide the necessary support, advice and guidance to everyone involved in your care.
Our aim is to prevent unnecessary admission or a placement breakdown and to avoid the need for an out of area placement.
If you are staying at The Brook we will work with you when you are leaving. We may provide a brief period of intensive support, alongside existing community services.
We cover the whole of Devon, including Torbay and Plymouth.

How to get help
How to get help
After a referral is accepted, someone from our team will be in touch within two working days.
- Before admission, our outreach team may get involved to offer advice or guidance to existing teams, or intensive short-term treatment, alongside other community and social care provision
- We may also review your current positive and proactive support plan (PPSSP)
- After discharge from The Brook, if needed, we can provide up to six weeks of extra help. This could include guidance for your care team or direct support to help with your recovery.
Referrals to our service are made via the South West regional single point of access.
