Our intent
We will embed an outward mindset approach in the way we work, break down silos and work as a trusted, equal partner across the system.
We will do this by
- Co-designing and co-delivering pathways with people who use services, carers and partners
- Strengthening locality-based working and professional fora
- Improving interfaces with primary care, local authorities, VCSE and acute providers
- Embedding integrated neighbourhood working, including mental health support in every community and expanding the partnership work with primary care and voluntary/ community sector organisations.
- Collaborating across the wider system, regionally and nationally to focus on population health outcomes, addressing inequalities in both direct provision and through continued development of provider collaborative(s) and alliances
- Work with academic, research and innovation partners to develop and adapt our services and offer to our communities, aligning priorities to improve outcomes
Success by 2030 means
- Smoother transitions and fewer people falling between services
