During your stay
During the first few days of your stay a series of assessments will take place that look in detail at your needs. This gives us, your family or carers time to consider plans for your care and treatment. You will be allocated a named nurse who will be your key point of contact during your stay.
Throughout your stay we will endeavour to work in collaboration with you. Your wishes will be listened to and you will have opportunities to be involved in discussions about your care.
Your care plan may suggest you take part in activities or groups, as well as individual work, depending on your needs. We encourage you to maintain a normal routine through structured activities.
If your admission allows, opportunities may include agreed trips off the ward.
Leaving Salus
Planning for your discharge commences soon after your arrival and is part of your care planning, so that appropriate services are in place when you leave.
If you are in staying in hospital under a section 3 of the Mental Health Act, there is a legal requirement to formally plan aftercare. This is called a section 117 planning meeting.
You will be seen by a community worker within seven days after discharge.
Depending on your needs, you may also be referred to the home treatment team, following discharge, to help support you for a short while.
Your continued wellbeing and recovery after you leave is very important to us and we will discuss the level of support you will receive.
