Learning disability primary care liaison service

  • We help and support people with a learning disability who are over 18 access mainstream health services.

    How we can help

    • We can support you if you find it difficult to access and engage with mainstream health services.
    • We can help you feel more comfortable and less anxious about certain medical tests or procedures, like blood tests or CT scans.
    • We can provide advice and support and signpost to appropriate services when required.

    When referred to our service you will see a primary care liaison nurse. They will meet you. You can have a friend, family member or carer with you, if you want to.

    We will complete your initial assessment to find out what help and support you need. Together we will develop your care plan that describes the help you need and how this is going to happen.

    When treatment is completed we will review your care plan again and you will be discharged from our service.

About our learning disability services

Our learning disability services support people with a learning disability when they see their GP, use primary care services, or when they go to the district general hospital.

We help those with mental health needs and challenging behaviour who can't benefit from the same adult mental health services as everyone else.

We provide assessment and treatment for people with profound intellectual and multiple-disabilities who have dysphagia needs and require postural care, who can't benefit from local mainstream therapy services.
