    How we can help

    We help people with a learning disability and very complex health problems to see if they are eligible for continuing healthcare NHS funding.

    One of our commissioning nurses will contact you or your representative to arrange to meet you. They will talk to you about the process and what information is needed and will identify if you have a primary health or social care need.

    We will work with people who become very ill and require a lot more care to set up 'fast' track' funding.

    People in a care home may be suitable for other funding.

    If you do receive funding we will ensure that your care needs are met. This would be done by listening to you, your representative and other health professionals. Together an appropriate care package would be set up to promote/maintain your health and to keep you safe, whilst enjoying life.

    If you are not eligible for funding then your social care worker is responsible for setting up your care package.

About our learning disability services

Our learning disability services support people with a learning disability when they see their GP, use primary care services, or when they go to the district general hospital.

We help those with mental health needs and challenging behaviour who can't benefit from the same adult mental health services as everyone else.

We provide assessment and treatment for people with profound intellectual and multiple-disabilities who have dysphagia needs and require postural care, who can't benefit from local mainstream therapy services.
