Triangle of Care Accreditation Achieved for Community Teams

The Triangle of Care is a model for collaborative care that involves the service user, staff member and a carer (or family member). It aims to promote safety, support recovery, and sustain wellbeing by acknowledging and including the important role of carers in the care process.
Joanna Duke, Together and Carer Lead, is pleased to announce that 12 of our community teams have achieved the Triangle of Care Star 2.
Jo said: "I'm delighted to share the fantastic news that a group of community teams have successfully achieved the Triangle of Care Star 2. This achievement recognises their strong commitment to improving the experience of carers and valuing their vital role in supporting individuals experiencing mental health and neurodiverse challenges. The following teams participated:

First Response Team (Exeter)
Devon Memory Clinic
TALKWORKS
Devon Adult Autism Interventions Team
Clinical Health Psychology and Neuropsychology Services
Overdose and Self-Injury Service
Community Forensic Team.

Special thanks to those who presented our joint report to the Carers Trust and in particular to our carers, Patricia and Debbie, whose contributions were highlighted in the report as both insightful and inspiring.

A carer said: "Although there still a long way to go, we have to recognise how hard it is to change culture around working with carers The work we are doing to improve how carers and families are involved feels very dynamic and that we're really moving forward with embedding it."
