Community Mental Health Framework

The Community Mental Health Framework (CMHF) implementation was delivered across Devon between 2021-2023.

A national programme, its aim was to improve community support and services for people with mental health problems to help ensure more joined-up and easier to access care and support. 

Here in Devon, Devon Partnership NHS Trust worked alongside Livewell Southwest to lead the changes as the statutory providers of community mental health services across the county, focusing on the needs of our population and working alongside those with lived experience. To do this, we worked alongside Devon's 31 Primary Care Networks, our local authorities and other organisations including housing providers and the voluntary, community and social enterprise sector (VCSE) via the Devon Mental Health Alliance (DMHA)

Although the implementation is complete, work is still ongoing to ensure the continued success of the Framework and its values, including a focus now on delivering the Personalised Care Framework. You can find out more about how we implemented the Framework in the below links.
