Intellectual Disability Community Forensic Service (ID CFT)

What we do
ID CFT, formerly known as the FIND service, offers a Post Diagnostic care pathway. The team work with people in secure care who have learning disabilities, neurodevelopmental disorders or acquired brain injuries to enhance their quality of care whilst in hospital and ensure that they only stay in hospital for as long as it remains the best option. They make sure that a person’s care pathway is appropriate, developing and supporting alternative care pathways if necessary, and providing training, support and supervision to staff to improve care pathways.

ID CFT are actively working with the South West Provider Collaborative, NHS England and Devon CCG on the Transforming Care programme which aims to improve the lives of adults with a learning disability and/or autism who display behaviours that challenge, including those with a mental health condition.
