Devon Community Forensic service (DCFS)

What we do
The Devon Community Forensic Team was established in 2018, being one of three sites awarded contracts by NHS England to become pilot sites for new community forensic teams. The Devon Community Forensic Team currently provides a service for people in the county of Devon (excluding Plymouth).  The focus of the team during the pilot has been on accelerating the discharge of patients from secure inpatient services, with a focus on supporting transition, reintegration in to the community and improving quality of life. In the future with continued funding from the South West Provider Collaborative we aim to expand the Devon Community Forensic Team building on achievements to date and to broaden the teams scope to prevent, where possible, admission and strengthen the resilience and capability of the wider health and social care system. 

The Devon community forensic team work as pairs (Occupational Therapist/Registered Mental Health Nurse) which provides a holistic approach working together to care co- ordinate people with a focus on relationships and social inclusion. We have a full MDT including a consultant psychiatrist, peer support worker, assistant practitioner, social worker and psychologist.
