We would like to gather feedback from our staff, people who use our services, carers and families to inform a new Quality Delivery Plan for our organisation. We've created a short survey which asks people a few questions like those below. Answer as many as you like or tell us your story in the relevant section. We are grateful for anything you share.
What are your thoughts on quality and improving services?
What do you want from the mental health and learning disability service you use or in which you work?
What is your best and worst experience of receiving or delivering care?
What would you do to improve services?
Please take this opportunity to reflect on the care you receive or, for our staff, the care you provide and help us to put quality at the heart of everything we do.
Click here to complete the survey, and thank you.
The survey will be open for three weeks, closing on 6 April 2016.
