Description
Our Primary Care Liasion Teams help and support people (over 18) with learning disabilities to access mainstream health services.
What the team offers:
- We aim to support people with complex health needs, who may find it difficult to access and engage with mainstream health services.
- We can offer help with de-sensitisation for particular health tests or procedures, for example, blood tests, CT scans.
- We can provide advice and support and signpost to appropriate services when required
