Devon Partnership NHS Trust has created a series of graphics to help colleagues working across health and social care when supporting an individual with their mental health.
The graphics have been tailored to each Primary Care Network (PCN) area in Devon – excluding those supported by PCNs in Plymouth.
The three simple slides aim to support colleagues and individuals seeking help by ensuring they receive the right care at the right time and in the right place.
The first slide illustrates the service offer available for those presenting with mental health support need. The second slide contains the contact details for each neighbour multi-agency team (MAT) including the MAT email address and meeting frequency, adult community mental health team (CMHT) and older adult community mental health team (OPMH CMHT) contact names and emails, plus contact details for eating disorder and rehabilitation services provided by Devon Partnership NHS Trust.
The third and final slide gives some basic presentation definitions to help colleagues further identify how someone may be presenting and what support may benefit them. The multi-agency team approach encourages discussion between services for advice and guidance and the MAT meeting function is to enable discussion where the right approach to meet needs in a timely way is unclear or may need a joint approach across services.
